We’ve come up with an interesting mystery in the Adobe InDesign Forum which has me stumped. Someone asked how they could add a new button to the Sample Buttons and Forms panel. I opened up the library panel and observed that it was locked. There was a lock icon at the bottom of the panel next to “77 of 77 shown”. All options for adding items to the library in the library , as well as icons for adding items are grayed out. This happens to the person who posted the question, as well as me on both my computers. But two other users who responded found that their Sample Buttons and Forms library is NOT locked. One showed a screen capture of how they could add a new button.
I did some research (Real World InDesign, and postings in this website). Kelly Vaughn did a great post this year on customizing that Library panel:

Her first screen capture showed the panel with a lock icon. But in subsequent screen captures, she seems to show that some things which are grayed out for me are showing up for her.
Can anyone help me solve this mystery? Why does the library appear locked sometimes (or to some people) and not to others?

I’m pretty sure you have to move the ButtonLibrary.indl file out of the application folder (Adobe InDesign > Presets > Button Library) and onto the desktop or some other folder in your Home folder. Edit it, then move it back.
Or, copy/paste it and rename it elsewhere, but then it wouldn’t open by default from the Buttons panel fly-out.
As a test I did the copy/paste routine and it worked fine, the lock was gone and I could add my own buttons.

Thanks, Anne-Marie. I’m not sure why the two other Adobe forum members who posted found this to be unlocked, however. (I’m referring to the Adobe InDesign Forum thread I linked to.)
