Form creation in Indesign

Hello everyone,
Need your help to know the task I’m going to do is a possible thing or not. I have Adobe Indesign CS6. I want to create a form. There are many check boxes. When I click on a 1 check box, I want to show a text box and add notes. If check box is not clicked, text box should be hidden. I can show you it to you. But I can’t find a place for any attachment. Can you please tell me this is possible or not in Indesign?

Let’s say you have 2 checkboxes: A and B
You can turn on the Hidden Until Triggered checkbox for object B (in the Forms & Buttons panel)
And then apply a Show/Hide Button action to checkbox A which shows B.
That will make B appear when you click A.
However, you cannot make one button show and also hide another object, so if the person turns off Checkbox A, then B will not hide again.

Also, note that hide/show will probably not work on any iPad/iOS PDF reader. They cannot handle buttons like this.
